Authorization To Release Remains
This document needs to be completed, signed, and faxed to Sneider & Sullivan & O'Connell's before we can transport the decedent to our facility.
Arrangement Preferences
This document is useful for preplanning funeral arrangments. Bring the completed form with you when you meet your Sneider & Sullivan & O'Connell's funeral director, or you may fax, email, or mail the form to your funeral director. All information will be held in strict confidence.
